Comments:
An unannounced monitoring inspection was conducted on 11/06/2018. Present during the inspection were 34 children, ages 4 years-12 years, 3 staff providing direct care and supervision and one support staff. The inspector reviewed 5 children's records and five staff records during today's inspection. The staff stated that there was no medication on site during the inspection. The children were observed in the following activities: an art/cooking project, large motor play in the gym, center/free choice play, having lunch and preparing to swim. The inspector reviewed the emergency procedures and emergency supplies during the inspection today. The inspector discussed the following with the staff: items from home and verifying children's ages. The inspector arrived at 9:50 a.m. and departed at 1:30 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on observations, record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled contained all of the elements as required by the standards. Evidence: 1. The record for Child #3 did not contain the health information as required by 22VAC40-185-140. 2. The record for Child #5 did not contain the phone number of the child's physician and did not contain complete addresses for the two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: The program coordinator updated the files during the inspection; got missing information from the parents.

Standard #: 22VAC40-185-70-A
Description: Based on observations, record review and discussion with the staff, the facility failed to ensure that staff records contain all elements as required by the standards. Evidence: 1. The record for Staff #1 did not contain the health information as required by 22VAC40-185-160. 2. The record for Staff #5 did not contain the documentation that two or more references as to character and reputation as well as competency were checked before employment, there was no record of reference checks being done, and the health information as required by 22VAC40-185-160.

Plan of Correction: The program coordinator will get the information right away.

Standard #: 22VAC40-185-350-E-4
Description: Based on observations, record review and discussion with the staff, the facility failed to ensure that whenever children are in care, the staff to child ratio is met for children from three years to the age of eligibility to attend public school, which is 1 staff member for every 10 children. Evidence: Child #5 was determined to be 4 years and 9 months old which required a staff to child ratio of 1 staff member for every 10 children; there were a total of 34 children in the direct care and supervision of 3 staff members during the inspection.

Plan of Correction: The program coordinator will require 2 days notice for parents to register a child for Fun Club to make sure there is enough staff to met the ratio. As of 5:00 p.m. 11/05/2018 there were only 16 children registered for the day.

Standard #: 22VAC40-185-560-G
Description: Based on observations and discussion with the staff, the facility failed to ensure that when food is brought from home the food is clearly dated and labeled in a way that identifies the owner. Evidence: Three lunch boxes were observed without a date and seven lunch boxes were observed unlabeled and did not clearly identify the owner.

Plan of Correction: All lunch boxes will be labeled at drop-off; for summer a new procedure will be put into place.

Standard #: 22VAC40-191-60-C-2
Description: Based on observations, record review and discussion with the staff, the facility failed to obtain the findings of a central registry search within 30 days of employment. Evidence: The personnel record for Staff #2 had a documented hire date for the licensed child care program of 10/01/2018; Staff #2 had been employed a different role with the organization previously. There was no record of the findings from a search of the central registry in the record for Staff #2. Staff #2 was observed working on 11/06/2018.

Plan of Correction: The program coordinator will submit today.

Standard #: 63.2-1720.1-A
Description: Based on observations, record review and discussion with the staff, the facility failed to ensure that all employees shall undergo a background check prior to employment and every five years thereafter. Evidence: The personnel record for Staff #2 had a documented hire date for the licensed child care program of 10/01/2018; Staff #2 had been employed a different role with the organization previously. The most recent sworn statement for Staff #2 was dated 04/23/2013. Staff #2 was observed working on 11/06/2018.

Plan of Correction: A new sworn statement was completed during the inspection.
