Comments:
The licensing inspector initiated and completed an unannounced complaint inspection in the center on November 6, 2018 in response to a complaint received regarding physical plant, special care provisions/emergencies, staffing/supervision and programs. Based on observations and interviews with staff conducted during the inspection, a preponderance of evidence does support the allegation regarding the area of programs, therefore the complaint is determined to be valid. Description: Based on observation and staff interviews, the licensee did not ensure that linens shall be clean and washed at least weekly. Evidence: On November 6, 2018, 2 of 2 mats and sheets were observed stored in a manner where they are touching one another. The area of the mat that had been touching the floor was observed touching the sheet where a child would be laying. The sheets were determined to no longer be clean.

Plan of Correction: The center director will begin today storing sheets, mats and blankets in individual bags labeled with each child's name.
