22VAC40-191 Background Checks (22VAC40-191)

Comments:
11/05/2018 Arrived 9:30am Departed 1:30 pm An unannounced monitoring inspection was conducted today. There were 44 children in care with 15 staff and two administrators. Children were observed eating, resting and playing outdoors. Previous violations, five children's files and 23 staff files were reviewed. Thank you for your cooperation. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on a tour of the classrooms areas, the facility failed to insure that all areas are are safe. 1. Two large lamps were in use in classrooms and were accessible to children in care. 2. One inch by one fourth inch pieces of wood are accessible to children on a low shelf in the toddler 1 room.

Plan of Correction: The Head of School removed the lamps and the wooden counting pieces during the inspection.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the facility failed to insure that the surface for diapering is nonabsorbant. Evidence: 1. A diapering mat in the bathroom and a diapering mat in the toddler room are torn, exposing foam padding.

Plan of Correction: The Head of School ordered new diapering mats.

Standard #: 22VAC40-191-60-C-2
Description: Based on the review of 22 staff records the facility failed to insure that all staff have completed all required background checks. Evidence: 1. There are no Virginia central registry clearances on file for staff #8, 13, 20 who have been employed more than 30 days.

Plan of Correction: The Head of Administration has reported that all the forms were submitted upon employment but have not been received from the central registry. New forms have been completed and mailed.

Standard #: 63.2-1720.1-B-3
Description: Based on the review of 22 staff files,the facility failed to insure that all staff have all required background checks. Evidence: 1. There are no out-of-state background checks on file for staff #16, 19, 21. All have lived in other states in the past five years and have been employed more than 30 days.

Plan of Correction: The Head of Administration has printed and distributed the forms. They will be completed and submitted by Tuesday.

Standard #: 63.2-1721.1-B-2
Description: Based on the review of 22 staff records the facility failed to insure that all staff have completed a fingerprint background check before beginning employment. Evidence: 1. There is no fingerprint background clearance on file for staff #16 who began work on 9-10-18.

Plan of Correction: The Head of School will insure that the background check is completed.
