63.2 Facilities & Programs.
An unannounced monitoring inspection was conducted on November 6, 2018 with center staff to determine compliance with the licensing standards. In the two year old room, there were 16 children present with three staff actively supervising. In the one year old classroom, there were 5 children present with two staff actively supervising. The children were having free play, eating lunch and preparing for rest. Lunch served today: hot dogs on buns, carrots, milk, grapes, and watermelon. Two staff files and five children's files were reviewed - please see Supplemental Summary for missing items. The last health inspection was conducted 7/11/2018. The last fire inspection was conducted on 6/27/2018. There are no medications on site today. The playground was not reviewed today due to inclement weather. Standard #: 22VAC40-185-140-A Description: Based on review of five children's files, the facility did not have documentation of a physical examination on file for each child within 30 days of attendance. Evidence: there was no physical on file for Child 4, who has been in attendance for more than 30 days. Plan of Correction: Center staff will request the information from the parent again.
Standard #: 22VAC40-185-40-G Description: Based on review of five children's files, the facility failed to meet the proof of child identity and age requirements as stated in 63.2-1809 of the Code of Virginia. Evidence: there was no documentation of having viewed the proof of identity and age for Child 1 on file. Plan of Correction: Center staff will request the information from the parents again.
Standard #: 22VAC40-185-240-D-5 Description: Based on staff interview review of two staff files , the facility failed to have documentation of at least one staff member on duty who has obtained daily health observation training in the past three years. Evidence: there was no documentation of any staff on duty with daily health observation training. Plan of Correction: Center director will schedule a daily health observation training class for staff.
Standard #: 22VAC40-185-500-B Description: Based on observation and staff interview, the facility did not have a leakproof diaper disposal system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: the foot pedal on the diaper pail in the twos restroom did not open the lid without a staff members hand touching the exterior. The diaper disposal in the infant nap room was not lidded. Plan of Correction: Center staff will fix the lid on the diaper pail in the twos bathroom and will get a foot operated diaper pail for the infant room.
Standard #: 22VAC40-185-550-D Description: Based on review of the emergency drill log and staff interview, the facility failed to conduct an evacuation drill each month. Evidence: there was no documented evacuation drill for October 2018. Plan of Correction: Center staff will resume evacuation drills this month.
