An unannounced monitoring inspection was conducted from 7:50 am-9:00. There were a total of 36 children with 2 staff members present. Children were observed arriving to the program, engaged in group activities and dismissal. A selection of staff trainings, children records, medications and other documents were reviewed. Standard #: 22VAC40-185-140-A Description: Based on record review, the center did not ensure that children's records contain a physical examination within 30 days of enrollment. Evidence: The record for child #1, enrolled 8/30/17 was missing a physical examination. Plan of Correction: Staff obtained a copy of the child's physical from the school office.
Standard #: 22VAC40-185-350-E-5 Description: Based on observation, ratios of one staff for every 18 children were not maintained for children ages 5 through 8 years old. Evidence: There were 2 staff present this morning. Staff #1 left at 7:55 am to take children to the bus, leaving Staff #2 with 29 children. When staff # 1 returned at 8:10 am, staff #2 had 36 children by himself. Plan of Correction: There was a staff that called out sick this morning. We will continue to try to call for a sub.
Standard #: 22VAC40-185-510-C Description: Based on a review of medication, the center failed to maintain parent's and physician authorization for long term prescription medication. Evidence: Child #2 had 2 prescription medications without written authorization from the parent and physician to administer Plan of Correction: Staff obtained a copy of the medication forms from the school office.
