Technical Assistance:
As discussed, when serving food for younger children, be sure to consider age appropriateness regarding the need to cut food into smaller pieces.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 95 children with 15 staff on site. Observation of all classrooms was conducted. The children were observed eating lunch and preparing for nap time. Lunch today was chicken nuggets, fresh sliced oranges or tangerines, salad and green beans. The classrooms and hallways of the center was decorated with seasonal artwork that the children completed. Paper plates were painted by the children and drying. Plans to make turkeys out of the plates are underway. Hand painted pictures using straws were completed by the children in the Inventors class. The infants were observed sitting playing with toys or held by staff. Observation of all crib areas was made. Staff presented knowledgeable of individual needs of the children such as allergies and diet preferences. Some staff interviewed regarding the center's emergency evacuation protocols. A total of 5 staff and 5 children's records were reviewed today. Previous violations from the April licensing inspection were reviewed and corrective actions were verified today. Standard #: 22VAC40-185-50-A
Description: Based on observation of the Sound Stage classroom, allergy information was not kept confidential. Evidence: Allergies for children were posted on the cabinet and available to the public.

Plan of Correction: We will place this information on the inside of the cabinet.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's records, two children's records were found incomplete. Evidence: Child #1's record has a social security card which does not meet the requirements for a proof of identity. Proof of identity requires proof of name and date of birth. Child 1's record is missing addresses for both emergency contacts. Child #4's record does not contain a proof of identity.

Plan of Correction: We will request to view birth certificates for both children and document the date and signature of staff who reviewed it.
