Online First Aid training (without a hands on element) is not acceptable.
An announced initial inspection was completed today. The physical plant (including measuring of the classroom), 5 staff files, 5 children's files, fire and health inspection, children's activities, emergency procedures, drills and supplies were inspected between 11:00am-1:30pm. There were 6 children under the supervision of 2 staff. Today, the children had circle time, learned about the weather and the letter of the day, played outside and enjoyed free play. The children bring in their lunches and/or snacks from home each day they are in care. All of the staff are certified in MAT (medication administration training). Once the background checks are reviewed by the Licensing Inspector, the license will be issued. Should you have any questions, please contact me at firstname.lastname@example.org.
Standard #: 22VAC40-185-60-A Description: Based on file review, 1 out of 5 children's records does not have all required information. Evidence 1. Child #1 does not have documentation of viewing proof of the child's identity and age. 2. Children #4 and #5 do not have the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. Plan of Correction: We will talk to the parents tomorrow.
Standard #: 22VAC40-185-70-A Description: Based on file review, 5 out of 5 staff records do not have all required information. Evidence 1. Staff #1 does not have documentation that two or more references as to character and reputation as well as competency were checked before employment. 2. Staff #2-5 do not have written information to demonstrate that the individual possesses the education required by the job position. Plan of Correction: The Director has contacted the staff about the education verification. The references will be filed
Standard #: 22VAC40-185-550-M Description: Based on file review, the child Injury reporting form does not include all required information. Evidence 1. The Injury report does not include: future action to prevent recurrence of injury or how the parents were contacted. 2. This form also has areas for multiple entries, which would prevent the confidentiality of information if used. Plan of Correction: The Director printed a sample form from the dss.virginia.gov website and plans to use this.
Standard #: 22VAC40-191-40-C-1-A Description: Based on file review, an applicant does not have the findings of a search of the central registry (SCR) on file. Evidence 1. Staff #1 does not have the results of a SCR on file. Plan of Correction: This has been sent to be completed.

Standard #: 63.2 Description: Based on observation, the facility failed to obtain the appropriate license from the Commissioner before operating a child day center. Evidence: 1. The licensee submitted an initial application to the Fairfax office. There were a few items which prevented the application from being complete. This information was sent to the licensee. Prior to the application being complete, the program began on September 4, 2018. Plan of Correction: The application has been completed. Once the completed background checks have been verified, the license will be processed.
