63.2 Facilities & Programs.
Today an unannounced 60 day visit was conducted between the hours of 2:45pm and 5:00pm. I observed 69 children directly supervised by 5 staff. Ratios and supervision were in compliance. Children were observed doing homework, listening to a story, playing with legos, playing table games, doing crossword puzzles, participating in group play, and playing on the playground. Staff were actively engaged with the children and assisting them with tasks as needed. The physical plant, 8 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Required staff training documents were on-site. Standard #: 22VAC40-185-510-C Description: Based on review of medications and authorization forms, the program did not obtain the parent or doctor's authorization for long term medications found on site. Evidence: The written medication authorizations forms for long term medications for child#1 and child#2 were completed on a Loudoun County Public School medication authorization form, this form does not authorize the CASA program staff to administer the medication. Plan of Correction: We will notify the parents that we need the form to authorize the CASA program, we will provide the forms.
Standard #: 22VAC40-185-510-G Description: Based on review of medications, it was determined that prescription medication was not maintained in the original, labeled container. Evidence: The long term prescription medication for child#3 was not in the original container with a prescription label attached. Plan of Correction: We will notify parents that they need to bring the original medication container.
Standard #: 22VAC40-185-550-M Description: Based on documentation reviewed, written record of children's injuries were found to be incomplete. Evidence: Two of two injury reports reviewed did not contain any future action to prevent recurrence of the injury, the date and time when parents were notified was also not documented. Injury report#1 dated 11/01/2018 and injury report#2 dated 11/02/2018. Plan of Correction: We will make sure we complete the form as required.
