An unannounced complaint inspection was conducted on November 15, 2018. The complainant reported concerns regarding programs and staffing and supervision. An inspection was conducted at the facility and observations were made in three classrooms. There was not a preponderance of evidence to support the allegation regarding staffing and supervision, therefore, that portion of the complaint was determined to be Not Valid. There was a preponderance of evidence to support the allegation regarding programs, therefore, that portion of the complaint was determined to Valid. Areas of noncompliance are identified on the Violation Notice.

Complaint related: Yes
Description: Based on observation and inspection of the facility, the licensee did not ensure that there shall be at least 12 inches of space between occupied cots. Evidence: During the complaint inspection conducted on November 15, 2018, during the designated rest period in the Three Year Old Classroom, there were multiple occupied cots with less than 12 inches of space between them. The cots measured approximately eight to 11 inches between cots.

Plan of Correction: Staff will be retrained on nap procedures and cot placement.
