Comments:
An unannounced monitoring inspection was conducted on Monday, November 19, 2018 from 10:10am to 2:00pm to review the center's compliance with licensing standards. The licensing inspector also conducted a focused inspection on this date to review operational responsibilities, forbidden actions, behavioral guidance, and background checks. During this inspection, the center's director was present and assisted the licensing inspector. The census for today's inspection consisted of 87 children in care and 15 staff directly supervising. During the inspection, the licensing inspector observed the children during free-play, singing songs, doing art projects, and playing outside. The children were also observed interactive with peers. Staff were observed having positive interactions with the children. The center has age and stage appropriate material and supplies. The following information was reviewed: program activities, supervision, staffing, and interactions with children, daily routines, menu, posted information, drill records, and agency inspection reports. The last documented health inspection occurred May 31, 2018. The last documented fire inspection occurred December 18, 2017. There were ten children's records and ten staff's records reviewed during this inspection. Standard #: 22VAC40-185-60-A
Description: Based on a review of 10 children's records on 11/19/2018, the center did not ensure to have documentation of child updates and confirmation of up-to-date information in the child's record as required by 22 VAC 40-185-420 E 3 for one out of 10 records reviewed. Evidence: 1) The record for Child #8 (DOE: 11/22/2016) did not contain documentation of child updates and confirmation of up-to-date information.

Plan of Correction: Corrected on site. Per the Director: The center will conduct updates twice a year, once in September and once in January to ensure all files are up-to-date.

Standard #: 22VAC40-185-290-3
Description: Based on observation on 11/19/2018, the center did not ensure that all electrical outlets shall have protective covers. Evidence: 1) In the Toddler B classroom, there were two electrical outlets observed by the Licensing Inspector without a protective cover.

Plan of Correction: Corrected on site. Per the Director: Management will ensure to check classrooms every other day after the cleaning crew has been on site.
