Comments:
A Subsidy Health and Safety inspection was conducted on November 20, 2018 from 7:15am to 9:00am to review supplemental health and safety requirements. There were 36 children (5-12yrs.) supervised by 5 direct care staff. The physical plant, 5 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed coloring, playing card games, board games, and computer games, and engaging in other free choice activities. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found. Description: Based on record review, 3 of 5 staff records did not contain documentation of completed Virginia Preservice Training for Child Care Staff. Evidence: Staff B's (date of hire: 9/2018), Staff C's (date of hire: 5/2018), and Staff E's (date of hire: 10/2018) records did not contain documentation of completed Virginia Preservice Training.

Plan of Correction: We will get their certificates and add them to staff files.
