The licensing inspector conducted an announced monitoring inspection on November 2, 2018 at the administrative office to review five staff records. The licensing inspector conducted an unannounced monitoring inspection on November 14, 2018 from approximately 3:40 PM until 5:30 PM. There were a total of 96 children present at the center with 11 staff supervising the children. During the inspection, children were observed completing homework, playing in the gym, eating an afternoon meal and having a dancing class. The afternoon meal consisted of Beefy macaroni, vegetable medley, apples and milk. All areas of the center including the cafeteria, gym, hallways and bathrooms were inspected. Playgrounds were not inspected due to inclement weather. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: emergency supplies, injury reports, fire and shelter-in-place drills, daily attendance, menu, required postings and parent agreements. Medication is administered and medication and medication authorization forms were reviewed. During the inspection, 10 children records were reviewed. Staff information required to be present at the center for staff members were reviewed. Standard #: 22VAC40-185-60-A Description: Based on the review of 10 children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #6 did not contain the addresses for the two emergency contacts listed in the child's record. Plan of Correction: Parent for Child #6 was asked for the emergency contacts while the licensing inspector was on site.
Standard #: 22VAC40-185-70-A Description: Based on review of five staff records, four staff records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the records for Staff #3 (hire date 01/10/17), Staff #4 (hire date 12/01/15) and Staff #5 (hire date 12/03/12) did not contain documentation of the required 16 hours of annual training. The record for Staff #3 (hire date 04/17/18) did not contain documentation of two character references prior to the staff member beginning employment. Plan of Correction: We will ensure all required items for staff files are obtained prior to staff starting and we will document the staff trainings as they occur throughout the year.
Standard #: 22VAC40-185-510-C Description: Based on review of medication and medication authorizations, the center did not ensure the authorization for medication for a long-term medication contained a physician?s signature. Evidence: During the inspection, the licensing inspector reviewed a medication authorization form for Child #7 did not contain a signature by a physician. A prescription medication was present at the center. Long-term prescriptions and over-the-counter medications required written authorization from the child's physician and parent. Plan of Correction: Parent for Child #7 was asked for the a copy of the form the doctor completed for the medication while the licensing inspector was on site.
