Comments:
An unannounced monitoring inspection was conducted from 10:15am 12:30pm. Upon arrival there were 23 children in care with 5 staff present. A sample of 4 children's records and 3 staff records were reviewed. There were no medications at the time of inspection. Children were observed during lunch and rest time.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on observation the center did not ensure children under 10 years of age were in actual sight and sound supervision of staff. Evidence: At approximately 11:25am there were 7 children under the age of 10 years napping in the room divided by a half wall on the right side of the main room. When the inspector walked into the classroom there were 7 children in the room napping that were not in sight and sound supervision of a staff member. Staff #1, who was responsible for the 7 children left the children for about 4 minutes before she returned back to the classroom. At approximately 11:27am Staff #2 left 8 children under the age of 10 years, napping in the room divided by the half wall on the right side of the main room. When the inspector walked into the classroom there were 8 children in the room napping, not in sight and sound supervision of a staff member. Staff #2, who was responsible for the 8 children left the children for about 2 minutes before she returned back to the classroom.

Plan of Correction: The following corrective action was provided during the exit interview: The program director will retrain staff sight and sound supervision.

Standard #: 22VAC40-185-550-C
Description: Based on observation the center did not ensure procedures/maps were posted in a location conspicuous to staff and children. Evidence: The center had 2 buildings on the property. The infant building did not have Emergency evacuation and shelter-in-place procedures/maps posted in the building.

Plan of Correction: The following corrective action was provided during the exit interview: The maps have been made, however, the program director neglected to post them in the building. They will be hung-up today.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a record review the center did not ensure all staff had the required fingerprinting. Evidence: Staff #3, hired 08/10/10, did not have a fingerprint. The Program Director verbally confirmed that Staff #3 has not had her fingerprint done.

Plan of Correction: The following corrective action was provided during the exit interview: An appointment will be made today with field print for a fingerprinting appointment. Staff #3 will not return to work until the results have returned.
