Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on November 27, 2018 from 9:45 a.m. to 1:30 p.m. There were eighty-two children were in the care of fifteen staff members in eight classrooms. Eight children's records were reviewed. Staff records were reviewed on August 15, 2018 at Bright Corporate office. The medication and medication paperwork were reviewed for two children. Areas of non-compliance may be found on the violation notice. Areas observed: hand washing/bathroom and diapering procedures, infant feeding, story time, outside play, free choice, art activities, lunch and nap. Interactions between children and staff were appropriate and the children seemed comfortable with the routine. The center is well equipped with a variety of age/stage materials for the children in care. Violations:
Standard #: 22VAC40-185-510-A
Description: Based on medication review and conversation with director the center failed to obtain written parental authorization for the administration of one prescription medication. Evidence: 1. diaper cream and authorization reviewed in classroom #5. 2. Child A did not have documentation of parent authorization for over the counter anti-fungal cream. 3. The director confirmed that there was no parent authorization.

Plan of Correction: The director will verify with the parents whether they still want that medication applied. If parents do wish this medication to be applied then the proper authorization will be obtained.

The director confirmed that there was no parent authorization.
