63.2 Facilities & Programs.
An unannounced monitoring inspection was conducted on 11/27/18 from 11:00am until 2:30pm. At the time of the tour, there were 44 children in care with 7 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed participating in teacher directed learning activities, eating lunch and resting quietly during nap time. Table sanitizing, hand washing, and restroom procedures were also observed. Medication, the emergency preparedness plan, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Two violations in the areas of administration and special care provisions and emergencies are identified on the violation notice and were discussed with the program director during the exit interview.
Standard #: 22VAC40-185-70-A Description: Based on record review and interview, the center did not ensure that staff records contain all the required information. Evidence: 1. The record for staff 1 contains documentation of only one reference check as to character and reputation as well as competency, where two are required. 2. The program director confirmed that the record for staff 1 contains documentation of only one reference check. Plan of Correction: A second reference check will be obtained for staff 1 today.
Standard #: 22VAC40-185-510-C Description: Based on a review of medication, the center did not ensure that procedures for administering medication shall include methods to prevent use of outdated medication. Evidence: 1. There were two asthma inhalers for two different children with expiration dates of January 2018 located in the center. Both expired inhalers had been used on the following dates: 11/9/18, 11/14/18 and 11/16/18. 2. The program director confirmed that the two medications were expired and had not been returned to the parent. Plan of Correction: The parent was contacted during the inspection and informed that new inhalers were needed. The expired inhalers will be returned to the parent today at pickup.
