Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, outdoor time, activities, group time, art, music, personal hygiene, lunch time, and rest time were observed. Children were observed; and staff and children files and other documentation were reviewed. There were 129 children, aged 2 months-5 years, found to be in care with 19 staff, and kitchen staff. Previous violations were reviewed, and had been corrected. There were 5 violations cited, in 5 separate areas of the standards. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 9:30am and concluded at 1:30pm.

Violations:
Standard #: 22VAC40-185-320-B
Description: Based on review of the rest rooms used by the children the facility failed to maintain compliance with this standard as required relating to availability of hand-soap. Evidence: In the first 2-3 year old classroom, on the left, there was no hand soap available for the children to use when attempting to wash their hands in the rest room or anywhere else within the classroom.

Plan of Correction: The director placed hand soap in the rest room for the children to use at the end of the inspection when violations were reviewed. Staff will be reminded to ensure their class room is compliant with standards at all times, and if items are needed from storage to request the item from the office personnel.

Standard #: 22VAC40-185-440-A
Description: Based on review of the classes serving children 12 months or younger the facility failed to maintain compliance with this standard as required relating to each child having a crib. Evidence: In the infant room, children aged 4-7 months, there were 7 children in care during the inspection and only 6 cribs. In the infant room, children aged 2-5 months, there were 8 children in care during the inspection and only 4 cribs. Children under 12 months must sleep in and have a designated crib while in care.

Plan of Correction: A request for maintenance to secure the extra cribs from storage and bring them to the facility had already been completed. Another request was made during the inspection as well. Cribs will be secured as soon as possible and placed in the both classes to ensure compliance.

Standard #: 22VAC40-185-550-D
Description: Based on review of documentation and discussions with staff the facility failed to maintain compliance with this standard as required relating to shelter-in-place drills. Evidence: There had been no practiced shelter-in place drills conducted, as of this inspection, for 2018 as required.

Plan of Correction: Both shelter-in-place drills will be conducted before the end of 2018 for compliance. Two drills will be conducted per year, and documented, for future compliance.

Standard #: 22VAC40-185-560-F
Description: Based on review of posted documents the facility failed to maintain compliance with this standard as required relating to a current menu. Evidence: The posted menu for the parents to see was for the previous week. The current weeks menu was not posted at he time of the inspection as required.

Plan of Correction: The current weeks menu will be posted by the end of the day today. To ensure future compliance the menu will be posted as required.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on staff file review and discussions with HR the facility failed to maintain compliance with this standard as required relating to out of state Central Registry requests. Evidence: Staff record #3, hire date 9-20-18, checked on the Sworn Disclosure that she had lived outside Virginia within the past 5 years. Based on this disclosure the facility was required to request a Central Registry Search for the state listed. As of this inspection, the request for this had not yet been initiated. The staff had been actively working at the facility.

Plan of Correction: HR had made an error and requested the out of state search on the wrong person. The request for staff #3 was initiated prior to the end of the inspection today. Upon receipt of the document, it will be forwarded to the facility and placed within the staffs record as required. Searches will continue to be initiated within the guidelines and time frames as required for compliance.
