63.2(17) License & Registration Procedures

Technical Assistance:
The facility's revised authorization for administration of medication form was reviewed with staff.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 9:00 AM and departed at 11:45 PM. Ninety three children were in care. Five classrooms were inspected. Morning arrivals, breakfast service, dental screenings and activities were observed. A sample size of five children's records and five staff records were reviewed. No children's injuries were reported. Medication administration was reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based upon review of five of five staff records, the facility has not ensured that documentation of a negative tuberculosis screening is submitted no later than 21 days after employment. Evidence: 1. The record provided for staff 4 indicates a date of employ of 9/10/2018. Documentation of a tuberculosis screening was not in the record provided. 2. The record provided for staff 5 indicates a date of employ of 8/27/2018. Documentation of a tuberculosis screening was not in the record provided.

Plan of Correction: The facility responded with the following:
1. File tuberculosis screening results in staff #4 records.
2. Immediate tuberculosis screenings for staff #4 and 5 and file in their records.

Standard #: 22VAC40-185-160-C
Description: Based upon review of five staff records, the facility has not ensured that documentation of a follow-up tuberculosis screening is obtained at least every two years. Evidence: The most recent documentation of a tuberculosis screening in the record provided for staff 3 is dated 11/30/2015.

Plan of Correction: The facility responded with the following:
Obtain the most recent tuberculosis screening results and file in #3 record.

Standard #: 63.2(17)-1720.1-A
Description: Based upon review of five staff records, the facility has not ensured that all employees have undergone a background check in accordance with 63.2-1716 requiring nationwide fingerprinting.
Evidence:
The record provided for 1, 2 and 5, all on duty with the children during the inspection, did not include the results of fingerprinting.

Plan of Correction: The facility responded with the following:
1. Ensure all staff have completed fingerprints
2. File updated fingerprint results for staff #1, 2, 5.
