Comments:
An unannounced monitoring inspection was conducted on 12/04/2018 from 3:55pm - 5:35pm. At the time of entrance there were 20 school age children in care with four staff. Children were observed during arrival from school. Afternoon snack, homework and reading time was observed. Records reviewed for eight children in care. Medication and emergency supplies reviewed. The violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, the center failed to ensure that staff emergency contact information was maintained on site. Evidence: 1. The emergency contact information for staff 1 was not maintained on site at the licensed program. 2. Administrative staff verified that emergency contact information for staff 1 was not on site.

Plan of Correction: Staff 1 completed an emergency contact sheet during the inspection.

Standard #: 22VAC40-185-510-E
Description: Based on medication review, the center failed to ensure that medication shall be labeled with the child's name. Evidence: 1. One box of over-the-counter medication was not labeled with the child's name. a. The medication was in a plastic bag that was labeled with the child's name.

Plan of Correction: The medication box was labeled with the child's name during the inspection.
