Comments:
Thank you for your assistance during this unannounced inspection. This on-site visit was conducted on November 13, 2018 from 1:30 p.m. through 2:45 p.m. This inspection was initiated from a complaint that was received by the Valley Licensing Office on October 31, 2018 regarding the allegation of supervision. Interviews were conducted with three staff, one children's record and the attendance sheet was reviewed. The information gathered during this inspection supports the allegation, therefore the complaint is valid. Areas of non-compliance may be found on the violation page.

Please complete the plan of correction within five business days and return to your licensing inspection.

Please contact me at 540-430-9256 if you have any questions and/or if I may be of assistance with licensing standards.

The violation notice was amended on February 27, 2019.

Staff #3 stated "they used to have that in their handbook, but it had recently been updated." 3.
