Description: Based on record review, staff did not receive the required training by the end of their first day. Evidence: Staff#1 did not have documentation of Orientation, Playground Safety Procedures, Recognizing Child Abuse/Neglect, and the Standards of Virginia for Child Care Centers.

Plan of Correction: I will make sure it is done

Standard #: 22VAC40-185-240-D-5
Description: Based on record review and interview with Director, there wasn't a staff member on-site with documentation of instruction in performing the daily health observation of children. Evidence; Staff#1 did not have documentation of daily health training.

Plan of Correction: I will find the copy and put it in the file

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous items were not in a locked place and inaccessible to children. Evidence: Plug in air-fresheners, which are hazardous and potentially toxic, are being used in the hallway and accessible to children. This was corrected at time of visit.

Plan of Correction: I will remove it

Standard #: 22VAC40-185-330-B
Description: Based on observation, resilient material under playground equipment does not meet minimum safety standards of 6 inches. Evidence: The resilient material (mulch and shredded tire) under the playground equipment is less than 1/2 inch.

Plan of Correction: I will look into getting it fixed

Standard #: 22VAC40-185-430-B
Description: Based on observation, materials and equipment accessible to children were not age and stage appropriate for the children. Evidence: Thumb tacks and pushpins, which are potential inhaling and choking hazards, were accessible to children in the Toddler room.

Plan of Correction: I will remove them
