Comments:
An unannounced monitoring inspection was conducted today from 2:40pm-5:00pm. There were 57 children (3yrs-10yrs) directly supervised by 19 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed arriving to the program, washing hands, playing with musical instruments, coloring, working on crafts, playing in the sand table, participating in group play, and eating snack. Today's snack consisted of yogurt and animal crackers. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-510-G
Description: Based on review of medications, a prescription medication was not maintained in the original, labeled container. Evidence: The prescription medication for child A was not in the original container with a prescription label attached.

Plan of Correction: We'll return the medication to the parents and request that they bring the medication in the original container.

Standard #: 22VAC40-191-60-C-2
Description: Based on staff record review and interview, the center did not obtain a central registry report within 30 days of employment for each staff member. Evidence: The files of Staff#1 (DOH: 08/06/2018), staff#2 ( DOH:09/04/2018), and staff#3 (DOH:08/20/2018) did not contain a central registry check result.

Plan of Correction: We'll do our best to locate the forms, otherwise forms will be submitted immediately to the appropriate agency.
