63.2(17) License & Registration Procedures
- Technical Assistance:
The licensing inspector conducted an unannounced monitoring inspection on Thursday, December 6, 2018 from approximately 10:30 AM until 12:20 PM. During the inspection, the licensing inspector observed two staff members supervising eight children. The children and staff were engaged in a variety of activities while the licensing inspector was on the premises. Children were observed playing with baby dolls, counting, building with blocks, singing songs and dancing, reading books, taking bathroom breaks and washing hands, eating lunch and resting at nap time. Lunch consisted of chicken, baked beans, mash potatoes, grapes and milk. All areas of the center including classrooms, hallways, bathrooms and outdoor play area were inspected. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: daily attendance, parent agreements, staff training information, required postings and menu. The center?s first aid kit and emergency supplies were inspected. Staff members stated medication was not present at the center. Standard #: 22VAC40-185-130-A Description: Based on review of five children's records and interviews, the center did not ensure three children submit proof of immunization prior to the children attending the center. Evidence: During the inspection, the licensing inspector observed the record for Child #1, Child #4 and Child #5 did not contain documentation of immunizations. Child #1, Child #4 and Child #5 was present at the center while the licensing inspector was on site. A staff member stated documentation still needed to be added and filed in the children's record. The immunization records could not be located while the licensing inspector was on site. Plan of Correction: We will contact the families of the children and request additional copies of the immunization record.
Standard #: 22VAC40-185-60-A Description: Based on the review of five children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #1 did not contain the address for one of the two listed emergency contacts. Plan of Correction: We will have the parent complete the child registration form when the parent picks up today.
Standard #: 22VAC40-185-550-D Description: Based on interview, the center did not ensure evacuation drills were practiced at least on a monthly basis. Evidence: During the inspection, the licensing inspector asked Staff #1 and Staff #2 if an evacuation drill had been completed for November 2018. Both staff members stated it had not been. Plan of Correction: We will review with the staff the requirements for conducting and documenting the required emergency drills.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on the review of two staff records and interview, the center did not request a search of out-of-state founded complaints of child abuse for one staff member who has lived outside of the state of Virginia within the last five years. Evidence: During the inspection, the licensing inspector observed the record for Staff #1 (hire date 12/17/16) indicated the staff member had lived outside of Virginia within the last five years. The record did not contain documentation of a request for founded complaints of child abuse from the state the staff member indicated she had lived in within the last five years. Plan of Correction: We will double check the other records to ensure this form has not already been requested. If it has not, we will request the search.
