63.2(17) License & Registration Procedures

Comments:
An unannounced renewal study was conducted from 9:30 am - 1:15 pm. There were 126 children in care being supervised by 20 staff members. The children were observed participating in story time, singing, and practicing for a holiday program. The infants were observed participating in gross motor play and playing with soft toys on the carpet. Lunch is provided from home. Ten children's records were reviewed and in compliance. Eight staff records were reviewed. Eleven medications were reviewed and in compliance. There are currently four staff members on site who are MAT (Medication Administration Training) certified. The most recent emergency escape drill was conducted 12/04/2018 and the most recent shelter in place drill was conducted 04/03/2018. Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of eight staff records it was determined that not all records contained an out of state central registry check for staff who have lived outside of Virginia within the last five years. Evidence: There was no documentation of an out of state central registry check for staff A who's sworn statement form (dated: 08/27/2018) indicated living outside of Virginia within the last five years.

Plan of Correction: Administration will request the out of state central registry check for Staff A.

Standard #: 63.2(17)-1721.1-A
Description: Based on review of staff records and agent records it was determined that not all central registry checks were repeated every five years. Evidence: The most recent central registry check on file for Agent A was dated 12/17/2012

Plan of Correction: Administration will request an updated central registry check for Agent A.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on review of staff and agent records it was determined that not all agents obtained an eligible determination letter from the office of background investigations before 09/30/2018. The record for Agent A did not have documentation of an eligible determination letter obtained from the office of background investigations.

Plan of Correction: Administration will request a determination letter from Agent A.
