Comments:
A Subsidy Health and Safety inspection was conducted on 12/10/18 from 10:00am to 1:00pm to review supplemental health and safety requirements. At the time of entrance there were 51 children ages five months old to five years old in care with 11 staff. Children were observed participating in various activities in the classrooms, eating lunch, and during rest period. Records were reviewed for five children and five staff. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. New subsidy requirements became effective on October 17, 2018. The facility has not yet complied with the following requirement(s): 22VAC40-665-540 (TB screening); 22VAC40-665-770 (procedures for emergencies). The Licensing Inspector reviewed this requirement with the facility to ensure future compliance.

Description: Based on a review of the food allergy list for the center and four children's records, it was determined that the vendor did not ensure that a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. The record for child #6 contained documentation from a physician that child #6 was allergic to peanuts. However, there was no allergy plan present for child #6 at the facility. 2. Staff #6 (Program Director) reviewed the record for child #6, and confirmed that no allergy plan had been received.
