Comments:
An unannounced monitoring visit and facility tour was conducted from 9:40am-11:05am. At the time of inspectors' arrival there were 14 children in care with 4 staff members. Children were observed participating in holiday program practice, Montessori lessons, transitioning between activities and hand washing. A sample of 5 children's records and 4 staff records were reviewed. The areas of noncompliance are detailed on the violation notice and were discussed with the Director during the exit meeting at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review it was determined the center did not ensure that each child's record contained all of the required components. Evidence: 1. The record for child #1(enrolled 9/5/17) did not contain the addresses for the two required emergency contact addresses. 2. The record for child #5(enrolled 9/23/16) did not contain the required proof of age and identity.

Plan of Correction: Director will request from the parents at pick up.

Standard #: 22VAC40-185-260-A
Description: Based on record review and interview it was determined the center could not provide to the licensing representative an annual fire inspection report. Evidence: The fire inspection documentation available was dated 9/19/17. The Director confirmed there was no new inspection documentation available during this inspection.

Plan of Correction: The Fire Marshall's office conducted an inspection.
