63.2(17) License & Registration Procedures
An unannounced renewal inspection was conducted with center director and center staff to determine compliance with the licensing standards. There were three infants and one toddler present with one staff. There were four preschool children and ten school age children present with two staff. The younger children were having rest time and the school age children were reading books and drawing. Snack served today: Cheez Its, peaches, and water. The playground was not reviewed due to snow. Six children's files and five children's files were reviewed - please see Supplemental Summary for missing items. The last health inspection was conducted 11/28/2018. The last fire inspection was conducted 11/30/2018. Standard #: 22VAC40-185-210-A-4 Description: Based on review of six staff files, the facility failed to have documentation of qualifications for each staff for the position hired. Evidence: Staff C did not have documentation of qualifications for the lead teacher position held on 12/12/2018. Plan of Correction: Center director will start the 24 hours of required lead teacher training this month.
Standard #: 22VAC40-185-340-F Description: Based on observation, the facility failed to have children under the age of ten in actual sight and sound supervision at all times. Evidence: the school age children were in the large room and the room off the kitchen in the main building. All children could not be seen or heard at all times. Staff E moved between the kitchen, the room off the kitchen, and the large room, but could not maintain visibility of all of the children. Plan of Correction: Center staff will position themselves in a way to observe all children at all times.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of six staff files, the facility failed to have each staff submit to fingerprinting prior to being hired. Evidence: Staff E was re-hired 11/04/2018, but did not get fingerprint results until 12/3/2018. Plan of Correction: Center director thought Staff E had previously been fingerprinted at re-hire. When documentation could not be found, Staff E submitted to fingerprint check.
