Comments:
An unannounced monitoring inspection was conducted today from 2:45pm-6:00pm. There were 23 children directly supervised by 5 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing outside, using computers, watching an interactive video, doing homework and eating snack. There was an abundant supply of materials for the children. Areas of the center were measured today for capacity calculations. Areas of non-compliance are identified on the Violation Notice.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, a child's record did not contain a physical examination by or under the direction of a physician. Evidence: Child#2's record did not contain the child's physical examination.

Plan of Correction: Physical exam records have been requested.

Standard #: 22VAC40-185-160-C
Description: Based on record review, it was determined that a tuberculosis (TB) screening or testing was not obtained for a staff member at least every two years from the date of the last screening or testing. Evidence: Staff#2's record contains a TB test/screening that expired on 11/13/2018.

Plan of Correction: Did the TB test for Staff#2, waiting on the results and report.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not maintain complete children's records. Evidence: Child#1's record is missing the father's work telephone number. Child#2's record is missing the father's work telephone number, and the street name and number of one emergency contact. Child#3's record is missing the street name and number of one emergency contact. Child#4's record is missing the mother's work telephone number. Child#5's record is missing the father's work telephone number, and one of two required emergency contacts.

Plan of Correction: We have revised all the documents to look for missing information and update the same.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff records, it was determined that staff records were not complete. Evidence: Staff#1's record did not contain documentation that two or more references as to character and reputation as well as competency were checked prior to employment.

Plan of Correction: We made phone calls to the references that were collected.

Standard #: 22VAC40-185-510-A
Description: Based on review of medication and interview, it was determined that there were two medications on site without a medication authorization form. Evidence: Child#1 has a long term nonprescription medication on-site without a medication authorization form completed and signed by the parent and the physician. Child#2 has a long term prescription medication on-site without a medication authorization form completed and signed by the parent and the physician.

Plan of Correction: Both children do not attend Nextide any longer.
