A Subsidy Health and Safety inspection was conducted on 11/19/2018 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from 2:45pm-6:00pm. There were 23 children directly supervised by 5 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing outside, using computers, watching an interactive video, doing homework and eating snack. There was an abundant supply of materials for the children. Areas of the center were measured today for capacity calculations. Areas of non-compliance are identified on the Violation Notice.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

New subsidy requirements became effective on October 17, 2018. The facility has not yet complied with the following requirement:
22VAC40-665-770. Procedures for emergencies. *Procedures for the continuity of operations to ensure that essential functions are maintained during an emergency must be included in the emergency response plan.
Standard #: 22VAC40-665-580-B Description: Based on record review and interview, it was determined that all staff who work directly with children did not complete the Virginia Preservice Training for Child Care Staff within 90 days of employment. Evidence: Staff#3 (DOH:01/30/2017) did not complete the Virginia Preservice Training for Child Care Staff training. Staff#5 (DOH:08/08/2018) did not complete the Virginia Preservice Training for Child Care Staff training. Plan of Correction: Staff identified are enrichment program teachers. We have asked them to complete the training by 1/25/2019.
Standard #: 22VAC40-665-580-E-1 Description: Based on record review, it was determined that a staff member did not have a current certification in cardiopulmonary resuscitation (CPR). Evidence: Staff#2, #3 and #4 have a CPR certification that expired on 11/14/2018. Plan of Correction: All staff have completed/renewed CPR certification.
Standard #: 22VAC40-665-580-E-2 Description: Based on record review, it was determined that a staff member did not have a current first aid certification. Evidence: Staff#2, #3 and #4 have a first aid certification that expired on 11/14/2018. Plan of Correction: All staff have completed/renewed first aid training.
