Comments:
The licensing inspector conducted an unannounced monitoring inspection on Thursday, December 20, 2018 from approximately 10:45am to 1:04pm to determine the center?s compliance with licensing standards. The center?s director assisted the inspector throughout this inspection. The census for today?s inspection consisted of 61 children in the care of 13 staff. During this inspection, the licensing inspector observed the children engaged in a variety of activities. The children were observed playing in centers, doing art activities, and during circle time. Later, the children were observed cleaning up from lunch and taking bathroom breaks. The staff were observed having positive interactions with the children. The required documentation was posted and included the license, last inspection notice, emergency numbers, allergy list, evacuation map/procedures, daily schedule and various parent information. The first aid kit and non-medical supplies were checked and found complete. The most recent evacuation drill was documented on 11/28/2018. The last shelter-in-place drill was documented on 06/20/2018 and 10/02/2018. The last documented health inspection occurred on 09/25/2018. The last documented fire inspection occurred 01/09/2018. Six staff records and six children?s records were reviewed during this inspection. Standard #: 22VAC40-185-130-A
Description: Based on a review of six children's records on 12/20/2018; the center did not obtain documentation that one of six children received the immunizations required by the State Board of Health before the child can attend the center. EVIENCE: 1) The record for Child #4 (DOE: 11/26/2018) contained immunization documentation dated 12/04/2018. The center shall obtain documentation that each child has received the immunization required by the State Board of Health before the child can attend the center.

Plan of Correction: Per the Center: Moving forward Kiddie Academy will make sure to have immunization records before or on the child's start date.
