Comments:
An unannounced monitoring inspection was conducted today, December 12, 2018 to determine the center's compliance with licensing standards. The Inspector initiated the inspection at 10:20 am and concluded the inspection at 12:00 pm. Upon the inspector's arrival there were nine children in care with three staff. Children were observed playing in centers that consisted of dolls, peg boards, farm animals and dress up. Children wore super hero outfits and told the inspector their super hero names. Children also had time with the tablets before bathroom breaks and hand washing were conducted to prepare for lunch. Children had lunch in the school cafeteria where staff sat with them and assisted with any help requested. Systems were in place for maintaining supervision and ratio during bathroom breaks. The classroom has age and stage appropriate materials and supplies. The parent board had the required information posted to include the license, emergency numbers, evacuation maps/ procedures, menu, daily schedule and various parent information. The facility's first aid kit and non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Three staff records and six children's records were reviewed. Staff have current CPR, first aid, daily health observation and MAT. Medications are administered by the elementary school nurse. Injury reports reviewed. The most recent evacuation drill was documented on 11/20/2018. One of the two shelter-in-place drills was conducted on 10/11/2018. A second shelter-in-place drill will need to be completed by the end of the school year. Health Inspection 09/04/2018 Fire Inspection 01/29/2018 Today's lunch consisted of cheese pizza, green beans, mixed fruit, jell-o and milk. Violations cited during the licensing period were checked for corrections. There was one repeat violation cited during today's inspection. Description: Based on review of children's records, the facility did not obtain documentation of a physical for each child before the child's attendance or within one month after attendance. Evidence: 1. The record of child #2, enrolled 09/04/2018, did not have documentation of a physical. 2. The record of child #6, enrolled 09/04/2018, did not have documentation of a physical.

Plan of Correction: Per the site director: we will contact the parent to have them provide the missing information.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's records, the facility did not obtain all required information for each child's record. Evidence: The record of child #6, enrolled 09/04/2018, did not contain the name, address and phone number of a second person to call in an emergency if a parent cannot be reached.

Plan of Correction: Per the site director: We will call the parent to obtain all the missing information.

Standard #: 22VAC40-185-420-E-3
Description: Based on review of children's records, the facility did not obtain documentation annually of parent confirmation that the required information in the child's record was up to date. evidence: The record of child #4 was last reviewed by the parent on 10/26/2017, exceeding the annual requirement.

Plan of Correction: Per the site director: I will send new forms to the parent to be up dated.
