The licensing inspector conducted a scheduled modification inspection on Tuesday, December 11, 2018. The provider is requesting to modify the license to change the capacity based on the use of additional space.. The request to modify the license was received 11/15/2018. Standard #: 22VAC40-185-70-A Description: Based on a review of staff records on 12/11/2018, the center did not ensure that each staff record contained the required information. EVIDENCE: The record of Staff #1, whose date of hire was 11/26/2018, did not contain documentation of: 1. the name, address and telephone number of a person to be notified in an emergency. 2. two or more references checks. 3. orientation training Plan of Correction: 1. Corrected on site. Had staff complete emergency contact information. 2. Will have Va. Workforce Alliance to send references to me. 3. Will complete orientation checklist today.
