Technical Assistance:
Technical assistance was provided about the new subsidy requirements that went into effect. The new requirements were discussed with center management.

A Subsidy Health and Safety inspection was conducted on 1/8/19 to review supplemental health and safety requirements. There were 60 children in care of 13 staff members. The children were observed playing with blocks, creating a collage, infants were observed sleeping and playing with toys. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on observation and staff interview, the center did not develop and implement a written policy and procedure to ensure care of children by consistent staff. Evidence: Staff # 1 stated that she was not aware of the updated subsidy requirements. A written procedure was not implemented at the center to maintain group sizes in each classroom.

Plan of Correction: We will work on a written plan and implement at the center. We will do small meetings with staff to discuss children's groupings and reasoning for primary care groups.

Description: Based on record review and staff interview, the center did not add a plan to ensure the continuity of operations during an emergency. Evidence: The emergency plan did not have a continuity plan added. Staff # 1 stated that she was not aware of the new requirements.

Plan of Correction: We will create and add a continuity plan to our emergency plan.

Standard #: 22VAC40-665-780-A-3
Description: Based on record review, lockdown procedures were not practiced annually. Evidence: The emergency drill records did not have any documentation to show that a lockdown drill was conducted. A lockdown drill has not been conducted within the last year.

Plan of Correction: We will conduct a lockdown.
