Technical Assistance:
Technical Assistance was provided related to the following: Medications, Annual trainings.

Comments:
An unannounced monitoring inspection was conducted from 10:20 am-1:45pm. 60 children were in care with 13 staff members present. Children were observed engaged in group activities using blocks, reading books and creating theme related collages. 5 staff and children records, medications and emergency drill records were reviewed. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, each staff member did not have documentation of a negative tuberculosis (TB) screening. Evidence: Staff # 1 stated that her most recent TB screening was done in Fall 2018. The documentation for the TB screening was not available on site for review.

Plan of Correction: I will get the copy of the TB and place in file.

Standard #: 22VAC40-185-350-E-2
Description: Based on observation, the ratio for one staff to 5 children was not maintained for children 16 months to 2 years of age. Evidence: 1. At 10:45 am the toddler room had 5 children present with Staff # 1. 2. Staff # 1 was over ratio with 6 children when another child arrived to the classroom, increasing the count to 6. Staff # 1 was over ration from 10:45 to 10:51 am, until a second staff person came into the room.

Plan of Correction: I will remind staff to let office know when they are close to being over ration so staff arrangements can be made.

Standard #: 22VAC40-185-510-C
Description: Based on observation, medication was administered after the medication authorization form expired. Evidence: 1. Child # 1's medication form was authorized by the parent until 1/4/19. 2. Staff # 2 administered the medication to Child # 1 on 1/7/19 which was past the medication's written authorization.

Plan of Correction: We will have the parent update the form and remind staff to not administer medication without required authorization.
