- Technical Assistance:
A Subsidy Health and Safety inspection was conducted on Friday, January 11, 2019 from 9:45 AM until 3:10 PM to review supplemental health and safety requirements. The following ratios were observed with a total of 71 children present during the inspection:
Infants - 12 children with 3 staff
Toddlers - 9 children with 2 staff
Twos (Discovery Preschool) - 17 children with 3 staff
Threes (Preschool) - 12 children with 2 staff
Fours & Fives (Pre-K) - 14 children with 2 staff
School Age - 9 children with 1 staff
The children and staff were engaged in a variety of activities while the licensing inspector was on the premises. Children were observed playing outside, learning about transportation, creating silly stories using the letter ?Kk?, building with blocks, playing with toys, being held and rocked, eating lunch and resting at nap time. Children were also observed having their diapers changed, taking bathroom breaks and washing their hands. Lunch consisted of turkey sandwiches, celery and carrots, pears and milk. All areas of the center including classrooms, hallways, kitchen, bathrooms and outdoor play area were inspected. The center was equipped with toys and supplies and items were available to the children. Seven children's and seven staff records were reviewed.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: 22VAC40-665-650-E Description: Based on interviews and documentation review, the center did not have a written policy that described how the vendor will ensure children receive care by a consistent staff or team of staff members.
During the inspection, the licensing inspector discussed the subsidy requirements that became effective on October 17, 2018. The facility has not written policy and procedure to ensure children are cared for by a consistent staff or team of staff members. This requirement was required to be in place by December 15, 2018.
Plan of Correction: We will develop a written policy to ensure group sizing and consistent staff for children in care.
Standard #: 22VAC40-665-770-B-5 Description: Based on documentation review, the center did not ensure the emergency preparedness plan contained continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence: During the inspection, the licensing inspector reviewed the emergency preparedness plan. The plan did not contain written plans on how the center will ensure continuous operation procedures are in place to ensure essential functions are maintained during an emergency. This subsidy requirement became effective on October 17, 2018 and was required to be in place by December 15, 2018. Plan of Correction: We will develop a written plan for continuous care in an emergency and add it to the emergency preparedness plan.
