Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(2)-70-A (Staff records); 22VAC40-185-(3)-210 (Program Leader qualifications)

Comments:
An unannounced monitoring inspection was conducted on 1/9/18 from 3:00pm - 4:15pm. At the time of entrance there were 11 school age children in care with two staff. One additional staff person and 11 children arrived during the inspection. Children were observed participating in various activities in the classroom, doing homework, and eating snack. Records were reviewed for five children for four staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of four staff records, it was determined that the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: 1. The record for staff #3 did not contain documentation of a negative TB screening. 2. Staff #1 (Program Director) confirmed that a negative TB screening had not been received for staff #3.

Plan of Correction: The facility responded: Staff #3 will be sent to complete a TB screening. All new staff will complete a TB screening within 21 days of employment.
