An unannounced renewal inspection was completed on 1/16/19. There were 11 children present with three staff supervising. Five children's records and three staff records were reviewed. Three emergency/as needed medications were reviewed for one child in care. The renewal applications have been received by the Division of Licensing Programs regional office; however, the licensing inspector has not received them to review and verify other records that may need to be reviewed for officers/agents listed on the application. The inspector arrived at the school at 3:15 pm and departed at 6:10 pm. The children were observed immediately after arrival, during homework time, during outside time, during snack, during movie time, during free table toy time, and during departures. There was discussion with the site director about the following: USDA requirements for snack even on special treat days.
Standard #: 22VAC40-185-140-A Description: Based on record review, the center failed to ensure that each child had a physical examination by or under the direction of a physician within 30 days of enrollment. Evidence: 1. There was no documentation of a physical examination in the record of child #4. Child #4 first date of attendance was 11/8/18. Plan of Correction: The site director will contact the parent to get a copy of the child's physical.
Standard #: 22VAC40-185-70-A Description: Based on record review, the center failed to ensure that staff records were complete with all required documentation. Evidence: 1. The record for staff #1 was missing a complete address for the emergency contact person for the staff. The record was also missing written information to demonstrate that the individual possesses the education or certification required by the job position. Staff #1 had a documented hire date of 12/19/18 and based on the job position would require certain education or certification for this position. Plan of Correction: This was corrected on the date of the inspection. The program coordinator at the main office called the staff person to get documentation of the emergency contact address. The staff person provided verification, by a photo of degrees received, of the education that would qualify the staff person for the job position.
Standard #: 22VAC40-185-240-D-1 Description: Based on record review and interviews, the center failed to ensure that there was documentation of Medication Administration Training (MAT) certification whenever the center has agreed to administer prescribed medications the administration shall be performed by a staff member or independent contractor who has satisfactorily completed a training program for this purpose approved by the Board of Nursing and taught by a registered nurse, licensed practical nurse, doctor of medicine or osteopathic medicine, or pharmacist; or administration shall be performed by a staff member or independent contractor who is licensed by the Commonwealth of Virginia to administer medications. Evidence: 1. The records of three staff were reviewed. There was no Medication Administration Training (MAT) certificates in any of the three records. 2. The program coordinator stated that staff #2 had completed the MAT course and practice demonstration training. 3. Staff #2 stated that she had completed the course on January 7, 2019. There was no verification of completing the MAT course to administer medications. Plan of Correction: The certificate will be placed in the staff person's file when it is received.
