Thank you for the time and assistance with the unannounced monitoring inspection that was conducted on November 30, 2018, December 17, 2018, and December 18, 2018. The inspection was the result of a self-reported incident that was received in the Valley Licensing Office on November 27, 2018. The on-site portion of the inspection was held on November 30, 2018 from 9:00 a.m. to 11:35 a.m. On that date there were 20 children present in one classroom and in the care of two staff members. Two staff members were interviewed, one child's record was reviewed and one staff record was checked for updates. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. Standard #: 22VAC40-185-350-E-4 Description: Based on staff interviews and verification from staff, the center failed to keep the required staff to children ratio of one staff member for every 10 children. Evidence: 1. On 11/26/2018, Staff Member A took Child #1 (age 4 years) out of the classroom for approximately five to ten minutes leaving Staff Member B as the only staff member with 18 children (ages 3 to 5 years). 2. Staff Member A and Staff Member B confirmed the classroom was out-of-ratio with one staff member in one classroom and caring for 18 children. Plan of Correction: MACAA Head Start supervision of child policy and the Virginia Department of Social Services Standards for Licensed Child Day Centers were reviewed with MACAA Fluvanna Head Start staff. The adherence to the policy and Standards will be monitored monthly to ensure compliance. Failure of staff to comply may result in further personnel disciplinary actions.
