- Technical Assistance:
Technical assistance today included medications, record keeping; physical environment; required postings. Consultation included new room arrangement. We also discussed the issue of a child with multiple food allergies with prior severe reactions, and an Allergy Action Plan dated 8/18, where the doctor authorized keeping the two medications at school but they have not been brought in since being returned due to an expired manufacturer's date.
An unannounced renewal inspection was conducted today from 11:00 am - 3:15 pm. There were 43 children present with staff. All rooms were within ratio, and more school-agers are expected later. I observed children busy with building projects, involved in dramatic play, listening to a story, getting ready for lunch and a smooth transition to naptime. Rooms have been rearranged, with new learning centers in all rooms. Records were well-organized. If you have any questions, please contact me at firstname.lastname@example.org
Standard #: 22VAC40-185-140-A Description: Based on a review of a sample of children's records today, one file did not have a physical exam accessible. Evidence: Child # 1, who began care 8/18, had a record of immunizations but did not have a physical exam available for review. Plan of Correction: I gave the form to the parent for her one year appointment.
Standard #: 22VAC40-185-260-A Description: Based on a review of records today, the center did not have a current fire inspection report. Evidence: The posted certificate expired 12/31/18, and the previous inspection was dated 10/26/17. Plan of Correction: The fire department was scheduled to come for an inspection on between 9 and 3 on January 9, but did not come. The appointment was scheduled in October. I will contact them today.
Standard #: 22VAC40-185-270-A Description: Based on observation during today's inspection, some areas were not observed in a safe condition. Evidence: A children's table in one room was observed to be balanced with one toy block to keep it steady. The diaper area in one room had areas where the wooden finish was deteriorating near the sink. A ceiling tile in the kitchen was not securely placed in the ceiling. Two crib sheets were observed not to stretch enough, leaving mattresses uneven. Plan of Correction: A work order has been placed for the table and to replace the diaper table. I put it the order for ceiling tile today. New crib sheets have been ordered, and will talk to staff about using appropriate sheets.
Standard #: 22VAC40-185-510-E Description: Based on a review of children's medication today, not all medications were labeled as required. Evidence: A bottle of an over-the-counter medication for Child # 2 did not have the dosage written on the bottle. Plan of Correction: Corrected during inspection.
