Comments:
The licensing inspector conducted an unannounced monitoring inspection on 01/22/2019 from 9:22am to 2:10pm. The children were observed participating in story time, playing in centers, reading books, and dancing. A formal observation was completed with the twos and pre-kindergarten classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the mini bus. Six children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. Ten injury reports were reviewed as well as the center?s injury prevention plan, which was updated 11/07/2018. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 12/31/2018 Last shelter-in-place drill: 10/20/2018 Last fire inspection: 03/07/2018 Last health inspection: 01/11/2019 Today, the following child to staff ratios were observed: Infants - 12:3 Toddlers ? 10:2 Twos - 16:2 Threes and Fours ? 18:2 Fours and Fives - 8:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-50-A
Description: Based on observation, the center did not ensure that children's information was treated confidentially. Evidence: The center's pre-kindergarten classroom had several children's allergy information posted in a classroom on a cabinet that can be seen by anyone entering the classroom.

Plan of Correction: We will place the allergy alert information in the classroom notebook.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the center did not ensure that one of five staff records had documentation of central registry results by the end of the 30th day of employment. Evidence: The record of staff #2 (start date: 7/30/2018) had documentation of central registry results dated 09/10/18.

Plan of Correction: We will ensure the check in process with DSS is documented clearly.
