- Technical Assistance:
Building repairs and renovations were discussed. Record Keeping and staffing were discussed.
A complaint investigation was completed at the facility. The Inspector observed classes, interviewed staff, reviewed records in response to complaints alleging ratios were over in the school during the afternoon. Based on evidence gathered, the complaint was deemed valid. Please call with questions 703-479-4679
Standard #: 22VAC40-185-340-D Complaint related: No Description: Based on record review, staff interviews and observation, it was determined there was not at least one staff member who meets qualifications of a lead regularly present with the children in the "PreK 1" group. Evidence: 1. Two staff were present in the "PreK 1" class and both could not answer questions related to the care of the children. Upon further questions, the staff stated they are assistants and the Inspector needed to ask management. 2. Records for both staff were reviewed and files revealed each were titled as assistants on the hire paperwork. 3. On November 13, 2018, the Inspector returned to the class and observed the same two staff present alone with the children. Plan of Correction: These staff were already placed in the class with a lead qualified who had to leave suddenly due to family issues. After we reviewed their paperwork and logged their training, we placed another staff with them that is lead qualified and are in the process of promoting one of the staff to a lead as the person meets the qualifications of a lead.
Standard #: 22VAC40-185-350-C Complaint related: Yes Description: Based on record review, interviews and observation, it was determined the staff to child ratio was not met in the two's group and the the three's class. Evidence: 1. Inspector observed on October 26, 2018 that there were 18 children with 2 staff in the three's class but two children were two years of age. The staff interviewed stated they have had children transition into the three's class before the children were 3 years old for at east two months. Records reviewed for found four instances in which ratio was over because there were over 16 children with 2 staff at the times when at least one two year old child was present in the group. The Inspector arrived again on November 13 and observed 17 children with 2 staff in the three's class from 3:00-3:30 and two children were two years of age. 2. On October 26, 2018, the Inspector observed 9 children with 2 staff in the toddler class but one child was under 15 months. Te ratio requirement for toddlers is 1 to every 5 toddlers but infants (under 16 months) have a 1to 4 ratio. Plan of Correction: We have been hiring and rescheduling staff.
Standard #: 22VAC40-185-350-E-3 Complaint related: Yes Description: Based on observation and interviews, it was determined the staff to child ratio in the two's class has not been met. Evidence: 1. On October 26, 2018, the Inspector observed the class and found 17 children with 2 staff. The staff were intervewed and stated they are short staff in the afternoon and try to work it out so ratios are met. 2. The Head count sheet was reviewed and the Inspector noted 6 out of 7 days had the two's class over ratio by 2 children during the times of 330-400. 3. Om November 13, the two's class was observed over ratio 1 and the record showed the class was over ratio on November 12, by 3 children. Plan of Correction: Not available online. Standard #: 22VAC40-185-560-M Complaint related: No Description: Based on observation, it was determined the staff did not sit with the toddlers during meal times. Evidence: 1. The Inspector observed the toddlers being served lunch and all three staff were standing while the children were eating. Plan of Correction: We have talked with all staff and had a meeting on issues . We are to ensuring each class has at least one staff member sitting with the children at a table during meals.
