Consultation on making sure medication authorization forms identifies the center for whom the parent and/or physician are granting permission to administer medication.
An unannounced renewal study was conducted today with the director, who was present with 66 children with 8 staff providing supervision. All the children except the infants were on their cots for nap time. Seven staff and six children's files were reviewed as well as five medications for 3 children. Staff reported that the children were served sausage pizza, salad, pineapple and milk for lunch. Two buses were inspected today however the playground was not inspected due to the rain. Please provide your plan of correction by 1/28/19. Fire inspection is dated 1/2/19 and Health inspection is dated 1/4/19. The inspection was initiated at 12:55 pm and ended at 3: 35 pm
Standard #: 22VAC40-185-140-A Description: Based on a review of six children's files and an interview with the director, it was determined that one child did not have a physical on file within a month of enrollment. Evidence: Child A enrolled on 11/26/18 and there was no physical on file. Plan of Correction: Family was notified that Immunizations were on file, but not physical. Parent contacted Pediatrician to resubmit physical to the center.
Standard #: 22VAC40-185-160-C Description: Based on a review of 7 staff files and an interview with the director, it was determined that one staff did not have a tuberculosis test/x-ray/screening every two years. Evidence: Staff A's chest x-ray was dated 10/10/16 and there was no documentation of a more current tuberculosis x-ray/test/screening. Plan of Correction: Staff provided the center with an updated TB Chest X-ray.
