Comments:
An unannounced Monitoring Inspection was conducted in the center on January 8, 2019. As reported, 18 children are currently enrolled in the program. The Licensing Inspector observed 17 children present with 3 staff members. The children's ages ranged from 3 to 5 years. The children were observed in their classroom and in the school cafeteria during the lunch meal. The children were observed as they participated in varied activities to include: breakfast, free play, story time and outdoor play. The children napped after lunch. Staff were observed interacting with the children. No changes in staff assignment. A sample of 6 children's records were reviewed. Documentation of the center's annual health and fire inspections was current. Documentation of the practiced emergency drills was reviewed. The first aid kit and emergency supplies were observed in the center. As currently reported, no medications are in the center. Staff have completed medication administration training (MAT). Findings of the inspection were reviewed with the Family Development Specialist, and the Lead Teacher at exit. The on-site inspection was conducted from approximately 10:30AM to 12:45PM. Thank you.

Standard #: 22VAC40-185-140-A
Description: Based on a sample review of children's records, the Center failed to obtain documentation to support that each child had a physical examination by or under the direction of a physician. Evidence: One of six reviewed children's records did not have documentation of a physical examination, the record for child #3 noted 08/29/18 as the first attendance date.

Plan of Correction: The parent has previously been notified and a copy of the physical examination has been requested, the responsible staff member will follow-up with the child's parent. Documentation will be maintained in the child's record.

Standard #: 22VAC40-185-150-B
Description: Based on documentation reviewed in a sample of children's records, the Center failed to ensure that a child's immunization form was signed by a physician, his designee, or an official of a local health department. Evidence: The reviewed immunization record in one of six children's records (child #3) was not signed by a physician, his designee, or an official of a local health department.

Plan of Correction: The responsible staff will review records closely to ensure all documents have the required signatures.
