An unannounced renewal inspection was conducted today January 30, 2019 to determine compliance with child day center licensing standards. The inspector arrived at 9:40 am and concluded the inspection at 1:50 pm. Upon the inspectors arrival The pre-school children were at the table reviewing letters and sounds. Children also worked on a worksheet identifying shapes. Later children played with toys and floor puzzles. The Infants were observed playing in the exersaucer and on the floor with rattles and board books. Toddlers had circle time where they reviewed flash cards of shapes and colors and sang songs. Diapering was observed in the toddler room along with hand washing. Today's lunch consisted of turkey, wheat bread, pineapples, corn and milk. After lunch children had nap time. Staff interacted in a positive and nurturing manner. All classrooms, rest rooms, kitchen and hallway area were inspected. The classrooms are equipped with age and stage appropriate materials for literacy, art, fine and gross motor skills and creative play. The facility had the required information posted to include the License, last inspection notice, emergency numbers, attendance sheets, allergy list, daily schedule, evacuation maps and various parent information. The first aide kit and non-medical emergency supplies were inspected and found complete for the building and the van used for transporting children. The inspector interacted with staff and children in care. Five staff records were reviewed during today's inspection along with the records of five children. Staff have current CPR, First aid training and daily Health observations. Per the director, medications are not being administered. Today's Ratio: (Infants): 3 children with 1 staff. (Toddler): 3 children with 1 staff. (Pre-school): 8 children with 1 staff. School age children were not is care during today's inspection. The last emergency evacuation drill was conducted on 01/25/2019. The two required shelter-in-place drills for the 2018 year were documented on 09/17/2018 and 10/11/2018. A lock down drill was conducted on 10/11/2018. The emergency preparedness plan is current. A complete renewal application has been received by the department. Standard #: 22VAC40-185-210-A-4 Description: Based on review of staff records and interview, the facility did not ensure that each program leader had the required training for the job position. Evidence: Staff #2, identified as a program leader and working in that capacity on 01/30/2019 did not have the six months of supervised programmatic experience as required by the job position. Plan of Correction: Per the director: I will have staff who meet program leader cover the class until 02/24/2019 when the staff will have completed six months of supervised programmatic experience.
Standard #: 22VAC40-185-550-M Description: Based on review of injury reports, the facility did not maintain a written record of children's serious and minor injuries in which the record included all required elements. Evidence: Six out of fourteen injury reports reviewed did not have the time parents were notified nor documentation on how the parent was notified. Plan of Correction: Per the director: I will train staff to complete the injury reports correctly with all required information documented.
Standard #: 22VAC40-185-570-C Description: Based on interview, the facility did not maintain a record of each child on formula that contained the brand of formula and the child's feeding schedule. Evidence: Child #6, an infant on formula did not have documentation of the brand of formula nor the child's feeding schedule. Plan of Correction: Per the director: I will create a form for parents with infants to fill out with the brand of formula or feeding method and schedule. I will make the list of formula available to staff in the infant class.
