Comments:
A Renewal inspection was conducted in the Center on January 11, 2019. The inspection was conducted from 11:00AM to 2:00PM. Staff reported there are currently 36 children enrolled in the program. On the date of inspection, 24 children were present. The ages of the children ranged from three to five years. The children were grouped in two classrooms with two staff in each room supervising the children's activities. The children were observed as they participated in varied activities which included: story time, outdoor play, nap, afternoon snack and dismissal at the end of the program day. Staff were observed interacting with the children. A sample of 8 children's records and 4 partial staff records were reviewed. Documentation of the center's annual fire and health inspections was current. No medications were observed in the Center. Staff have completed medication administration training (MAT). Documentation of the practiced emergency drills were reviewed. Emergency supplies and first aid kits were observed in the center. In addition to background checks for board officers and agents, 5 complete staff records were reviewed at the central office on January 17, 2019. Findings of the inspection were reviewed at exit. Standard #: 22VAC40-185-270-A
Description: Based on direct observations, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition. Evidence: The fence gate leading into the outdoor playground had wire loose from the frame, wire was sticking out at the bottom of the gate.

Plan of Correction: A maintenance request has been submitted to report the loose wire, the wire will be reattached to the gate frame.

Standard #: 22VAC40-185-320-B
Description: Based on inspection, warm water was not provided at the classroom sink which was used by the children for hand washing. Evidence: The LI turned the water on, only cold water was connected at the sink.

Plan of Correction: A maintenance order has been submitted to have the portable sink repaired and in operable condition.
