Comments:
An unannounced monitoring inspection was conducted today. There were 51 children in care, with a total of 5 staff members assigned to their direct supervision. Children's records, children's medications, classroom programming, emergency preparedness items, the physical plant, and children's nutrition were inspected today. The children were observed eating lunch from home, creating art projects, playing games, and taking a neighborhood walk. This inspection was completed between approximately 11:00am and 12:15pm. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record B does not include authorized pick ups information. 2. Child record C does not include: the mother's work phone number; the parent's annual review of the child's file. 3. Child record D does not include the father's place of employment.

Plan of Correction: The files will be updated.

Standard #: 22VAC40-185-510-A
Description: Based on a review of children's medications, it was determined that one medication was on site without the required authorization. Evidence: One child's prescription medication was on site without a medication authorization form completed by the parent and physician.

Plan of Correction: The child just started and the parent was already asked to bring the authorization form. I will follow up with the parent.
