Comments:
An unannounced monitoring inspection was conducted with the director beginning at 12:20 PM and ending at 2:30 PM. There were 23 children, ages ranging between 11 months through 4 years old, in care. The infants were observed being bottle feed, rocked by caregivers, and playing with toys. The older children were observed cleaning up after lunch and during nap time. Bathroom and hand washing procedures were also observed. Eight children's records and four staff records were observed and in compliance. The emergency escape drill was reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent shelter-in-place drill was documented as 9/24/18. The most recent emergency escape drill was documented as 1/3/19. The most recent fire inspection was dated 12/21/18. The most recent health inspection was dated 8/16/18. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, not all of the areas inside the center were maintained in a safe condition. Evidence: At the top of the stair case between the three and four year old classrooms there was a metal hinge hanging out creating a scratching hazard. In the toddler room there were three table top seats that did not have the safety straps installed in them.

Plan of Correction: The director stated that the table will be removed from the center by the end of the day. The hinge was removed during the inspection.

Standard #: 22VAC40-185-550-D
Description: Based on a review of documentation, the center did not conducted two practice shelter-in-place drills per year as required. Evidence: There was only one shelter-in-place drill documented during the 2018 year.

Plan of Correction: The center will ensure that two shelter-in-place drill will be conducted and documented every year.
