63.2(17) License & Registration Procedures
An unannounced renewal inspection was conducted with center staff on February 6, 2019 to determine compliance with licensing standards. There were 109 children present with eight staff actively supervising. The children were arriving from school, having afternoon snack, and playing indoor group games. Snack served today: cheerios and milk. Three staff files and five children's files were reviewed - please see Supplemental Summary for missing items. Evacuation drills have been conducted monthly and the last shelter in place drill was conducted 9/26/2018. Five emergency medications for four children were reviewed - please secure long term medication authorizations for medications that will be on site more than 10 days. Standard #: 22VAC40-185-140-A Description: Based on review of five children's files, the facility failed to have documentation of a physical within 30 days of the child's first day of attendance. Evidence: Child 4 did not have documentation of a physical on file within 30 days of attendance. Plan of Correction: Center staff will request the physical from the parents.
Standard #: 22VAC40-185-350-C Description: Based on observation, the facility failed to have the correct staff to children ratio applicable to the youngest child in the group. The ratio for preschool children is 1 staff person for every 10 children. The ratio for young school age (ages 5 to 8) is 1 staff for every 18 children. Evidence: there were 15 preschool children and 25 young school age children (K, 1st, and 2nd) with two staff supervising. A third staff person had stepped out of the room to take two children to the rest room. Four staff were needed to maintain ratio. Plan of Correction: Center staff will be mindful of ratios when combining groups of children for activities.
Standard #: 22VAC40-185-510-C Description: Based on review of five medications for four children, the facility failed to have a current parental authorization for medication that was renewed after 10 work days or a long-term prescription written authorization from the child's physician and parent for each medication. Evidence: the short term medication authorization (STMA) (completed by parent only) for Child A's medication was completed 1/17/2019; the STMA form (completed by parent only) was completed for Child C on 1/15/2019, and the STMA form (completed by the parent only) for two medications for Child D was completed 1/15/2019. Plan of Correction: Assistant director will request this information from the parents today.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of three staff files, the facility failed to have documentation of a fingerprint check for each staff hired prior to January 2018 no later than October 1, 2018. Evidence: Staff 1 (date of hire 10/11/2016) and Staff 3 (date of hire Sept 2017) did not have documentation of having completed the fingerprint check. Plan of Correction: Staff 1 and Staff 3 will submit to fingerprinting as soon as an appointment can be scheduled.
