Technical assistance was providing regarding confidential postings, retention of birth certificates, and parents walking children directly to classrooms.
An unannounced monitoring inspection was conducted today from 9:00am-12:10pm. There were 43 children present at the beginning of the inspection and were directly supervised by 8 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, emergency supplies, and first aid kit were inspected. Children were observed reading books, playing outside, engaging in circle time activities, singing, eating snack, and free play. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of materials for the children. The director advised there is no medication on site today. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-70-A Description: Based on record reviews, staff records were not complete. Evidence: 1. Staff #4 (Date of Hire: 10/1/17) did not have documentation that references were checked prior to beginning employment. Plan of Correction: I have checked the references for this staff. I can call the references again and document it.
Standard #: 22VAC40-185-240-C Description: Based on record review, not all center staff have attended the required 16 hours of annual training. Evidence: 1. 3 of 5 staff records reviewed did not contain documentation that 16 hours of annual training were completed. 2. Staff #1 did not have any documentation of training for the previous year. Staff #4 had 14 hours of training. Staff #5 had 11.5 hours of training. Plan of Correction: This year, I will see that the staff complete 16 hours of training.
Standard #: 22VAC40-185-270-A Description: Based on observation, equipment outside of the center was not maintained in a safe and operable condition. Evidence: 1. On the toddler playground, the red bench has deteriorating wood in the seat area. When pushed, the seat easily bent with pressure and exposed a small crack. This poses a hazard to children in care if they were to sit on it; the wood would break and splinter. Plan of Correction: I will call the carpenter and get the bench fixed. In the mean time, I will move the bench from the playground.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances, such as cleaning materials, were not kept in a locked place that prevents access by the children in care. Evidence: A Clorox wipe container and Lysol spray bottles were located in an unlocked closet in the Montessori 3 classroom. There were 10 children present in the classroom at the time. Plan of Correction: It has been locked up.
Standard #: 22VAC40-185-350-E-1 Description: Based on observation and record review, the center did not maintain a ratio of 1 staff member to 4 children for children from birth to 16 months. Evidence: 1. At the time of the inspection, there were 5 children present in the Toddler 2 classroom with 1 teacher present. The youngest child is age 14 months and requires a ratio of 1 staff to 4 children. Plan of Correction: We will see that somebody else replaces staff when they leave the classroom to keep it in ratio.
Standard #: 22VAC40-185-550-M Description: Based on record review, the center did not maintain a complete written record of children's injuries. Evidence: 1. 2 of 5 children's records contained injury reports that did not include future actions to prevent recurrence of injuries. Plan of Correction: We will see that staff complies with this requirement.
