An unannounced investigation was conducted November 19th 2018 and November 20th 2018 in response to a self- reported incident regarding sight and sound supervision. During the investigation, staff were interviewed, staff files were reviewed, children's files were reviewed and the classroom and adjacent bathroom was observed. The preponderance of evidence does support the allegation; therefore valid findings have been found in response to this incident. Violations can be found in this report. Standard #: 22VAC40-185-340-A
Description: Based on interview with staff, when supervising children staff did not always ensure their care protection and guidance. Evidence: 1) Child A (3 years old) was left in the bathroom unsupervised by a staff member. 2) Staff A said when Child A was returned to the classroom she was "crying". 3) Staff B said when Child A was returned to the classroom she was "visibly upset".

Plan of Correction: All staff were retrained on sight and sound policies as well as face to name sheet documentation.

Description: Based on interviews with staff, a child under 10 years of age was not in actual sight and sound supervision of staff. Evidence: 1) On November 19th 2018 Staff A took Child A (3 years old) to the bathroom which is located approximately 5 feet beyond the threshold of the classroom. Staff A left Child A inside bathroom unsupervised for approximately five minutes. A non-staff member returned the visibly upset Child A to the classroom. 2) Staff A stated that she was standing between at the entrance of the classroom to supervise both the children in the classroom and Child A, who was in the bathroom. Staff A stated that she was "distracted by a disagreement that occurred in the classroom between two children at the puzzle table". Staff A leaves the area where the bathroom is located to de-escalate the disagreement at the puzzle table. 3) According to Staff B, Staff A said she was taking Child A to the bathroom. Staff B stated " I looked at the clock and told the children it was 15 minutes until lunch". Staff B stated "Lunch time is 11am". Staff B stated "at 10:50 Child A was brought in" by a staff member of the adjacent childcare facility.

Plan of Correction: New policies have been put in place to ensure children are monitored at all times. Face to name sheets are used during all transitions that occur outside of the classroom. All staff have been trained on transitions and sight and sound regulations
