Standard #: 22VAC40-185-510-E Description: Based on a review of medications, the center failed to ensure medications were labeled with the dosage amount. Evidence: Over-the-counter medications and the parental written authorizations for two children did not identify the dosage amount (children a. over-the-counter medication) (child c. a prescribed medication and an over-the-counter medication). Plan of Correction: The Program Director will contact each parent for a review of the authorizations and medications. The PD will ensure the documentation is complete and the medication is labeled with the dosage amount.
Standard #: 22VAC40-185-510-G Description: Based on a review of medication and documentation of the administration record, the Center failed to administer a prescribed medication in accordance with the prescriber's instructions pertaining to dosage, frequency, and manner of administration. Evidence: The LI reviewed a medication for child e. and the child's medication administration record (MAR). The MAR indicated the medication was administered every 4 hours on the following dates (12/14/18 @11:15am and 3:15pm; 2 puffs), (01/15/19 @ 10:20am and 2:20pm; 2puffs) and (01/17/19 @ 10:10 am and 2:02pm, 2puffs). The record noted that the parent had requested the medication to be given scheduled every 4 hours, no conditions or symptoms were noted on the MAR. The pharmacy label on the prescribed medication indicated that the medication was to be administered 2 puffs every 6 hours, as needed for asthma-like symptoms. Plan of Correction: The Program Director will review the center's medication policies with all medication administration trained staff. The Staff will follow physician orders for all medications. The parent will be contacted to be informed that the Center will follow physician's orders for administration, and if a change is requested, a new physician's order must be in the center prior to the administration of the medication.
